Services for People with Identified Needs

Camp Knutson

A camp for youth and adults with medical conditions and identified needs. We partner with other organizations to host camps for youth and adults with identified needs. Situated on a beautiful north-woods chain of lakes, Camp Knutson is also a retreat facility September through May.

Care Coordination Services

Through a partnership with health care plans, care coordinators work with individuals to promote access to services and resources that help them prevent future hospitalizations and stay healthy in their homes.

Employment Services

Employment specialists help people with identified needs find meaningful work based on their individual interests and goals. Support can include skill and resume-building, mock interviews and on-the-job training.

Financial & Legal Services

Services that support the financial and legal health of individuals with identified needs, including financial counseling, FMS in-home support services, guardian and conservator services, pooled trust, and power of attorney.

Residential & In-Home Support

Housing and support options for individuals with identified needs, including traditional supported living homes, family Host Homes/shared living options and specialized housing for individuals with significant behavioral or medical challenges.

Resources for People Supported
